Community-based diabetes education, support groups and self-management classes for African Americans
In 1999, Public Health—Seattle & King County received a grant from the Centers for Disease Control and Prevention for a Racial and Ethnic Approaches to Community Health (REACH) 2010 Demonstration Project, an initiative to mobilize communities to eliminate health disparities among racial and ethnic minorities. As a member of the REACH coalition, CMCH organized and hosted a diabetes summit for African American men and women who are living with diabetes and conducted focus groups that informed the development of a plan for a community-based initiative to reduce diabetes-related health disparities in the African American community. As the lead agency for the African American community, CMCH organizes community-based diabetes education, support groups and selfmanagement classes in Seattle-King County.
